How to Layer Laser Skin Treatments for the Best Results – Coveteur07.25.21

Laser treatments are popular for a variety of skin goals. "Layering treatments, also known as combination therapy, can be a powerful method to maximize benefits, minimize adverse effects, and even save time for clients," says Sam Pang, an aesthetic nurse practitioner at SkinSpirit.

Dr. Julius Few, a board-certified plastic surgeon, considers these interwoven, non-surgical treatment combinations to be "the future of aesthetics." He calls them "Stackable Treatments," a term he coined that calls out their synergistic and complementary usage. "By layering innovative peels, injectables, laser treatments, ultrasound technology, and advanced skin care, the 'Stackable Treatment' approach simultaneously targets the three major components of aging: skin wrinkling, loss of volume, and sagging," Dr. Few tells Coveteur.

"You're already going to be down once, so let's go ahead and take advantage of the downtime and enhance the effect of all the treatments," reasons Dr. Corey L. Hartman, a board-certified dermatologist. "As long as we're not doing anything that will be detrimental to the other treatment or too aggressive [for the skin], we know that we're going to get an added benefit from the synergy of different technologies used at one time." This might mean opting for microneedling, which promotes collagen production in the skin, alongside radiofrequency, which tightens the skin by stimulating elastin production.

Ahead, we speak to skin-care experts in order to break down four of the best layering treatments that'll have the greatest impact on your skin.

"My favorite treatments to layer at the same visit are Fraxel or Clear and Brilliant, PRP injections, and microneedling," says Pang. "This combination boosts collagen, improves the look of fine lines, pores, [and] pigmentation, and gives the skin a healthy glow."

Lasers, like Clear + Brilliant, create tiny channels in your skin that allows topical products, like your serums and moisturizers, to penetrate deep into your skin, says Dr. Hartman. "That's why it's an ideal time to apply PRP," he adds. "We know PRP is great for stimulating the growth of collagen and elastin. When we enhance that effect and get it deeper in the skin, we know we will get a more robust effect." Alsothere are two forms of the Clear + Brilliant laser. If decreasing the appearance of hyperpigmentation is a skin goal for you, be sure to seek out the Clear + Brilliant Permea.

The microneedling promotes "maximum collagen stimulation with reduced risk of post-inflammatory hyperpigmentation while being safe for all skin types," according to Pang. This combined therapy is ideal for rejuvenation and maintenance, and the downtime isn't badyou should be fully healed in five days or less.

For a more aggressive treatment to address deeper creases, smooth out rough texture, or to treat acne scars, consider leveling up to the Fraxel laser. "When treating acne scars or deeper wrinkles, I like to combine Fraxel with microneedling," says Pang. "This allows for an aggressive treatment with a reduced risk of negative outcomes." She adds that downtime will include two to four days of intense redness and swelling, with lingering pinkness for roughly 12 days.

When it comes to removing pigment, tightening the skin, and promoting collagen, Dr. Emer's Signature Bright and Tight uses a radiofrequency and microneedling device to stimulate collagen above and below the surface of the skin. This is followed up with the BBL Hero by Sciton; it's an intense pulsed light (IPL) treatment that treats surface-level pigmentation from sun damage, so it's ideal for eliminating unwanted browns and reds in the skin. Plus, the BBL Halo treatment can be used to smooth texture and fade acne scars.

After this treatment, PRP is applied like a serum to the skin in order to promote healing and reduce inflammation. Oh, and alsoif you're thinking about trying out this treatment combination, it's important to keep in mind that IPL treatments aren't recommended for people with melasma or those with deeper skin tones.

"For melasma, pairing a PicoSure laser with PRP, and then following up with a good topical regimen in between treatments, is a great way to manage it," Dr. Hartman says. Because of melasma's risk of being triggered by heat, you may want to avoid IPL treatments, Fraxel, and Clear + Brilliant. PicoSure is the only laser with FDA approval to treat melasma because there's a smaller chance that heat or light will negatively affect your skin.

"We have also been experimenting more with microneedling," says Dr. Hartman. "It might be beneficial for treating melasma; there's been an increase in the number of studies that are beginning to look at the effectiveness of microneedling." He also notes that microneedling has helped to bring color back from hypopigmentation, aka the loss of pigment due to sun damage or tattoo removal.

According to Dr. Ben Talei, a board-certified plastic surgeon, he's seen the most significant skin improvements from the Tri-L treatment. It's a combination of a radiofrequency skin-tightening treatment, laser resurfacing, and plasma therapy, he explains. This trio of treatments works on the deepest layers of your skin, as well as its surface, to address lines, wrinkles, and pigmentation.

Towards the end of the procedure, your blood is drawn and fat is taken from under the skin on your abdomen to create a serum that's then injected back into your skin, several layers deep, explains Dr. Talei. Tri-L definitely isn't for the faint of heart and it requires a bit more TLC post-procedure than other treatment combinations (the CO2 laser alone requires up to 10 days of downtime.) However, according to Dr. Talei, after three months, you'll notice tighter skin along with improvements in pigment and texture.

"It's important to remember that not every treatment can be combined," Pang says. However, when the right treatments come together, they can help treat an array of specific skin goals. Depending on your skin tone, type, diagnosis, and desired outcomes, your aesthetic nurse, dermatologist, or plastic surgeon will be able to recommend the appropriate course of treatment for you.

It's also important to remember that taking care of your post-laser skin-care routine matters as well. The regimen that you follow at home long after your sessions have ended is what will determine the long-term success of your treatments. (And, yes, that includes being diligent with your applying, and re-applying, sun protection.) But for devoted skin-care lovers, the efforts they take are undeniably worth it.

Ross said: There is nobody else doing this business in North Cumbria at the moment and it can benefit people greatly as I come to you, instead of people going to a shop, or if they are disabled or have trouble with mobility and transport I can help those people as well.

Rosss business is called Rethink My Ink and is currently open at standard business hours as well as evenings and weekends if customers are too busy working on weekdays.

Ross joined the Cumbria Chamber of Commerce New Enterprise Allowance Scheme that gives new start-ups the ability to be funded and help make their professions or craftsmanships a legitimate business.

When Ross decided he wanted to do tattoo removal as a profession he wanted to make sure he would be trained properly.

Training in Barrow, Ross became certified by the British Medical Laser Association and The Laser Training Academy, assuring customers there is no better qualified member to remove their tattoos

Ive always been fascinated with tattoos, he said, and I love working with people and getting to know them.

I noticed a gap in the market in Cumbria and the opportunity that there is for this sort of thing and thats why I decided to launch the business.

The 50-year-old said his customers have been over the moon with the work he has done so far and are surprised by how quick and efficient he is.

Ross hopes to expand his tattoo removal business in the future.

He said: It would be great to eventually open a shop in Cockermouth so that we have the mobile side to the business and the walk-ins for the shop as well but I need to create a bigger client base first. Offering both aspects to the business would be ideal because there may be some people who want to come to a shop and have that experience whereas there are others who, for instance, cant for any reason. But we will always have our mobile facility open all hours.

Are you the not-so-proud owner of some regrettable ink and considering tattoo removal? Welcome to the club. I have four (five, if you count the one on my back as two) tattoos from my late teens and early twenties that I could probably definitely do without. So I set out to finally part ways.

If you've heard anything about laser tattoo removal, it's probably along the lines of it being insanely painful, like maybe even more than getting the tattoo in the first place. But while there's plenty of info on what to consider before getting a tattoo (and pages on pages of enticing inspo), there still isn't a whole lot of discussion surrounding the dark side of ink jobs: What happens if you grow to no longer love that little shooting star or random Latin phrase (ahem, see below)?

While going through the process, I picked up a handful of tips along the way that I wish I'd known going into it. So to do you all a solid, I put together a list of everything I've learned. From the costs associated to the potential side effects, here's what you need to know about tattoo removal.

I'd previously had one tattoo zapped at a spa (I was living in small-town Canada where there weren't plastic surgery offices or dermatologists), where an aesthetician used an outdated heat laser that ended up burning and scarring my skin. This time around, I got my treatments done by John F. Adams, M.D., at the New York Dermatology Group, where everything is done under medical supervision. I suggest you find your own removal expert by asking friends, influencers, or even by stopping people that you see with removal in processwhich, yes, I have done.

Tattoos don't just disappear after a once-over with the laser. (I wish!) A complete tattoo removal takes a minimum of 2 1/2 years on average, says Bethany Cirlin, tattoo removal specialist and owner of Clean Canvas More Art. Laser treatments should be scheduled three months apart from one another so you get the most out of each treatment. This allows your body to break down as much of the tattoo as it can while also giving your body the opportunity to heal completely before your next session."

As of writing this, I've had six sessions, and I'd wager that I need about five more, despite the fact that my initial estimate was six to eight sessions. It takes a long time to complete because each time the tattoo is lasered, particles are broken down and digested by the body's immune system. The regeneration period is up to eight weeks, and the next time you go, the laser breaks down new particles of pigment. And so on and so forth.

If you have your procedures done by a doctor, the bill for each visit can run you hundreds of dollars. Brace yourself: The American Society for Aesthetic Plastic Surgery estimates the average cost per session at $463. But see point number one for why it's worth it. Your tattoo removal cost can also will vary based on the size, color, and age of your tattoo.

Contrary to the old belief that light, colored ink was hard to remove, Adams assured me that all hues will now disappearno matter your skin tone. (FYI: The previous explanation was that, similar to laser hair removal, the laser would solely be attracted toward dark colors, like black.) With PicoSure technology, he says you can even get out yellows and greens, which were previously the most stubborn.

Once you know you're unhappy with a tattoo on your body, immediately start using a zinc oxide sunscreen on it, says Cirlin. The most common reason people can't get lasered is because their tattoo has had sun exposure. By using a zinc oxide anytime you're outside, you'll help protect your tattoo, which will allow you to get lasered regardless of the season."

While some laser sessions are quick and easy, not all are. Mine have been taking about 45 minutes because we take before photos, clean the areas, inject them with lidocaine for freezing, laser them, ice them, and then bandage them. Oh, and sometimes a weird side effect happens where I taste metal when the laser hits my skin. Adams says it's a sensation that some people experience when the lidocaine is hit by the laser and that it's totally normal.

But consider this warning from Cirlin first: Pain is completely individualized, and if you tell someone that something is going to hurt, they go into it with that expectation. That said at my practive, we offer a topical numbing cream, which helps to take the edge off the procedure." Just know that even with a numbing cream though, your experience may not be totally pain-free. "We also use a piece of equipment called a chiller that uses cold air to help keep our clients comfortable, she says. It's definitely worth asking for a consultation with your practitioner ahead of time if you're worried about pain.

I would advise that you budget for discomfort for about a week. For me, the sites blister and need to be covered in a salve and bandaged for a few days; then they start to depuff, scab, peel, and regenerate. There is good news though: The more treatments you have, the less after care there is (since there's less ink reacting to the laser).

You'll need things like Aquaphor, bandages, and even clothes that don't rest on your tattoo. Yes, I actually bought shirts that didn't have material where my neck/back tattoo is. At first, I was cutting tags out, but when tagless cotton tees still made the spot hurt and itch, I figured keyhole backs were a good investment. If you have tattoos on your ribs or feet (and like to wear bras and shoes) it might be best to plan your sessions accordingly.

It's important that you stay out of the sun and avoid tanning beds before your laser tattoo removal. Don't shave or wax the tattoo area.

If possible, stop smoking several weeks ahead of your first laser tattoo removal session because smoking can delay your healing.

Although the Nd:YAG laser is extremely effective, it's not an instant treatment. Most tattoos require a series of treatments for optimal results. Each treatment is scheduled 6-8 weeks apart, so you can see the full outcome before having the next treatment.

For the first day after laser tattoo removal, use an ice compress to alleviate any discomfort. You should avoid getting the treatment area wet for as long as your dermatologist recommends, usually around 24 hours.

Your dermatologist will likely suggest an antibiotic ointment, which you should apply as directed for at least several days after your laser treatment.

You may develop small blisters or scabs as you heal. Avoid picking at them; doing so can delay your healing and interfere with your results.
